Download original Texas Motor Carrier Application

Complete the form and fax or mail it back with the payment indicated on the application.

Wait a few days and when you receive your Certificate Number, give it to Carter and Company. If we have a current qualifying Auto Liability policy for you, we will order the Form E be sent to the Texas Department of Transportation electronically and you should be ready to get your cab cards in a few days.
